The focus of the strategy is about how Knowsley Primary Care Tust, and Social Sevies staff together with Knowsley Council and other stakeholders central to the delivery of the Long Term Conditions Strategy make the best use of all the resources at their disposal to make a real difference to the people with Long Term Conditions.
The images of young and old people showed ‘promotion of independence’ – making the right things easier to do. Also, the importance of psychological well being of Long Term Condition patients and increasing satisfaction in NHS Service usage. The theme of ‘graph paper’ represents how many Long Term Conditions are monitored and all artwork was done to strict NHS Guidelines.
